PEDAL EDEMA AND KNEE PAINS

 This is an online e log book to discuss our patient de-identified health data shared after taking his / her / guardians signed informed consent. Here we discuss our individual patients problems through series of inputs from available global online community of experts with an aim to solve those patients clinical problem with collective current best evident based input.

I have been given this case to solve in an attempt to understand the topic of “patient clinical data analysis” to develop my competency in reading and comprehending clinical data including history, clinical findings, investigations, and come up with a diagnosis and treatment plan.


Pedal edema  since 1 year

B/l knee pain since 1 year
Bloating sensation since 4 to 5 months of abdomen
Ear discharge since 1 year
Patina was apparently asymptomatic 1 year ago then he had complaints of pedal edema upto the knees ,insidious in on set,gradually progressive  in nature,pitting type on and off since 1 year
Relieved on walking at night aggrevated on sitting for long time
No c/ o Shortness of breath  on exertion  ,orthopnea, pnd,chest pain,palpitations, oliguria and facial puffiness
B/ L knee pain since 1 year aggrevated on walking Relieved on rest
Patient was on Tab Lasix since 2 to 3 months stopped since 5 days
Past history 
K/c/o Hypertension  since 1 year is on regular medication 
Not a k/c/ o DM, CVA,CAD,TB,ASTHMA, EPILEPSY 
Ear discharge present since 1year occasionally Relieved on medication. 

Systemic Examination
P/A soft and non tender 
No organomegaly
CVS S1 S2 heard no murmurs
RS BAE + NVBS heard
CNS No focal neurological deficit

Regular Alcoholic for 45 years  stopped since 1year





Treatment 
1.TAB ISOSORBIDE DINITRATE +HYDRALAZINE HYDROCHLORIDE (20MG+37.5 MU)
2.TAB TELMISARTAN 40MG/PO/OD
3.TAB CARDIVAS 6.25 MU/PO/BD
4.TAB DYTOR  PLUS (10+25MG) PO/BD
5.TAB ATORVASTATIN +CLOPUDOGREL (20MG +75 MG) PO/HS
