Patient came to casualty with complaints of Shortness of breath since 2 am
HISTORY OF PRESENT ILLNESS
Patient was apparently asymptomatic 10 days back then dhe was taken to local hospital because of generalised weakness
She also developed Shortness of breath-Grade 4 MMRC Insidious in onset gradually progressive aggravated on lying down
No relieving factors
Complains of b/l pedal edema ,pitting typr extended upto knee joint and no aggrevating or relieving factors
Complains of decreased urine output since 3 to 3days (related to decreased oral intake)
C/o chest tightness ,facial puffiness since 1 week
No c/o chest pain, palpitations, giddiness, sweating
No c/oof tingling sensation of hands and feet
Patient is K/C/O Diabetes mellitus type 2 since 15years on regular medication
Using Tab Glicazide 60mg OD
K/C/ O Hypertension since 15 years on regular medication
( Tab clinidipine 10mg OD)
K/c/ o Chronic kidney disease on conservative management since 10monthd
Not a k/c/ o CVA,TB,ASTHMA, EPILEPSY
Systemic Examination
P/A soft and non tender
No organomegaly
CVS S1 S2 heard no murmurs
RS BAE + NVBS heard
CNS No focal neurological deficit
