A 48 year old male with shortness of breath and chest pain.

A 55 year old male daily labourer by occupation came with chief complaints of 
Shortness of breath since 3 days
Chestpain since 3 days
HOPI
Patient was apparently asymptomatic
4years ago he was diagnosed with Hypertension 
 1 year ago where he complained of pedal edema decreased urine output 
And experienced similar complaints 1 month ago got  admitted.
3 days ago he complained of SOB which was associated with chestpain which was of dragging type throughout the day.
Progressed Grade  2 to 4 NYHA
Orthopnea present
1 session of dialysis done yesterday night 7 30 pm
Past history 
Diagnosed with diabetes and Hypertension.

Personal history:
Diet -mixed 
Appetite-Less
Bowel and bladder-regular 
Sleep-Disturbed 
Alcoholic from 25yrs and he stopped 5yrs back and occasional smoker.

GENERAL EXAMINATION:
Pallor -present 
Pedal edema-present 
No signs of cyanosis,icterus,clubbing,lymphadenopathy


SYSTEMIC EXAMINATION:
CVS - s1s2 heard 
RS -normal vesicular breath sounds heard 
Abdomen  - soft,non tender
CNS- no abnormality detected
Provisional diagnosis 
Chronic renal failure
